A Patient Guide:

At Goyt Valley Medical Practice we provide a wide range of medical and health services. We aim to ensure that every patient receives the medical management they need from the most appropriate healthcare professional.

Our healthcare professionals work at both our sites, which means that you can be seen at the surgery that offers you the most convenient appointment. Please note that not all services are available at our Chapel-en-le-Frith surgery.

Out of Hours Care

If you require urgent medical attention when the surgery is closed then please call 111 and you will be transferred to the out of hours service. This is a centralised service and you will either be given medical advice or directed to the most appropriate service.


As well as offering both pre-bookable and book on the day routine GP appointments we have nurse practitioner services who offer on the day appointments for new problems. The nurse practitioners are highly qualified clinicians who can diagnose, prescribe and refer if required. They also operate a triage system for both home visits and urgent problems.

In addition we offer telephone consultations with our nurse practitioners and GPs for results and advice and with our in-house pharmacist for medication queries and problems.

Further to the above we offer a range of clinics including:

  • Family planning
  • Antenatal
  • Diabetes
  • Cardiovascular
  • Respiratory
  • Hormone replacement therapy and osteoporosis
  • Mental Health

Booking an appointment

If you would like to book an appointment for any of the services that we offer then please contact the surgery on 01663 735 112. A member of our reception team will then book you an appointment with the most appropriate clinician. You can also come into the surgery to book an appointment but it is unlikely that you will be seen straight away unless you problem is urgent.


If you need to order a repeat prescription then you should drop your request into either surgery, indicating from which local pharmacy you would like to collect your prescription.

Alternatively you can sign up to order repeat prescriptions on-line. If you would like to sign up for this then please call into either surgery, bringing a form of photo ID with you. Please note this service is not available for patients under 16 years of age.

Please allow 2 working days before collecting your prescription.

Registering with the practice

We need you to complete as fully as possible the enclosed purple GMS1 registration form and also the enclosed confidential questionnaire and Summary Care Record form. Return all of them to the practice as soon as possible to enable us to put your details on our system. We cannot register you without all the information.

If we feel you would benefit from a personal face to face appointment we will contact you.

Self-Referral to Physiotheraphy

Information for patients:

If you are registered with Goyt Valley Medical Practice, you can now access NHS physiotherapy by making a self-referral without having to consult your GP.

To make an appointment:
Telephone the physiotherapy department at Cavendish Hospital, Buxton on 01298 212 811.

Please note:
·        You must be age 18 or over to self-refer. If you are under the age of 18, please obtain a referral from your GP.
·        Your NHS number is essential for acceptance of your referral can be found on your NHS card or please ask at reception if you do not have this information.

Use of data

All patient information is considered to be confidential and we comply fully with the Data Protection Act. All employees have access to this information in relation to their role and have signed a confidentiality agreement. Information may be shared, in confidence, with other NHS organisations in the interest of patient care. The organisation will strictly apply the rules of confidentiality and will not release patient information to a third party without proper valid and informed consent, unless this is within the statutory exempted categories such as in public interest, in which case the reasons for it will be individually and specifically documented and authorised by the responsible clinician.

Please note that it is practice policy to record all telephone calls for the purposes of patient and staff care, security and dispute resolution. Recordings and their use will be at the Partners’ discretion and will comply with the Practice’s Data Protection registration.
